We offer two ways to access your medical record:

Paper Copy of Medical Records:

Please complete the following steps to obtain a paper copy of your medical records:

  1. Print and complete the Medical Records Release Form.
  2. Complete, sign and date the form. In order to verify your identification and validate your authorization, we require that you include a legible copy of a valid photo I.D. (e.g., driver’s license, military I.D. or state I.D.).
  3. Mail, fax or personally deliver your paperwork to the facility where you were treated.

Please note: Medical Records copied for reasons other than continuity of care are subject to a copy fee (CA Health & Safety code 123110(b)).

Please allow 5-10 days for processing from date of receipt of the completed authorization. Federal law permits up to 30 days.

Walk-In Hours

Please bring the completed authorization form and your photo ID to the HIM/Medical Records Department. The department is open Monday through Friday from 8:00:00 AM to 5:00:00 PM.

Urgent Requests, Records for your Physician

For immediate continuity of care, your healthcare provider can request records.

The physician office must fax a written request on their letterhead to (951) 788-3175 indicating the patient’s name, date of birth and date of visit in the facility. For assistance call (951) 788-3165.


Insurance, Attorney, Disability and Third Party Requests

Requests should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address below. We cannot accept faxes and email requests.

Per CA law, birth registries must be completed within 10 days from the date of the birth of the baby. You can find out information on how to request a birth certificate  by visiting your local Office of the County Clerk Recorder. If you have specific questions regarding a birth registry that is in process, please contact the Facility Birth Clerk at (951) 788-3791.
